[CED/CD/EAT] CED Monthly Reports

Greetings, CEDs!

At our last CED Retreat in December, we had a lengthy conversation about the CED Monthly Reports that get submitted.  Over the years, they became very inconsistent and sporadic with submissions.  We discussed updating and automating with reminders as well as simplifying the form.  I have incorporated the feedback from that conversation and compiled a new version of the report submission form.  Here are a few of the changes:


  *   Programs - this section is staying as is.
  *   Personnel - this section has been removed as it was usually combined with "Pending Issues."  The thinking is that if there are any Pending Issues or with Personnel, those would be brought to our attention sooner than in a monthly report.
  *   Budget - this section has been removed for similar reasons as "Pending Issues" and "Personnel."
  *   Pending Issues - see "Personnel"
  *   Because of... - this section remains but is changed (described below)

New fields are:

  *   County - this is the only information CEDs need to select as I've related it to another table in the database to help with automations.
  *   Reporting Month - this changed slightly.  But I can pull the Month and year from a native field in the database to isolate the actual reporting month to keep track.
  *   Highlights (formerly "Because of...) - I changed this based on the feedback from the CEDs and added some help text to discuss what we're looking for here.
  *   Events - this is to callout any major events that should be shared with EAC so that we can consider in our travels across the state, or meeting with elected officials in those counties.
