Subject: TA/RA Conversation Forms and Annual Review

Dear all,

Grad students and faculty, please complete the following two forms:
      

  1.
Conversation Form:

     *
If you were a TA this semester, please complete the TA conversation form attached. The first step is for the student to fill it out on their own. Then the student and the faculty member must meet to go over the document and it must be signed by the instructor.
     *
If you were a RA this semester, please complete the RA conversation form attached. As above, the first step is for the student to fill it out on their own. Then the student and the faculty member must meet to go over the document and it must be signed by the instructor.

Please complete either form and return signed via email to me by May 1st.

Annual Progress Form:

Please note the instructions on the form:
     *
1. Student completes first section, sends to Advisor before each annual committee meeting (adding pages as needed).
2. After the committee meeting, Major Advisor adds a summary (on additional pages as needed), includes any comments from the committee, and forwards to the graduate coordinator.
3. GSPC reviews.
*Note: Annual Committee Meetings are required by May 15 of each year.

Please complete this form and return via email to me by May 16th, the sooner the better.
