[Plantsci] Zoom Phone will be replacing desk phones campus wide

Dear Employees,

UITS is planning to migrate the phone service in the School of Plant Sciences to Zoom Phone. If you have an individual work phone number, after migration you will use the Zoom app Phone feature for your external calls, in addition to the internal extension call feature. (See Zoom Phone project information<https://it.arizona.edu/zoom-phone-project>.)

As a first step, information needs to be collected by September 6, 2022.

Who Needs to Fill Out the Form
You work for the School of Plant Sciences in Forbes, Marley, or Herring Hall and:

  *   You have a University phone number (i.e., 621*/626*, NOT a cell phone) and need to keep your phone number for external calls and/or shared line services as a job requirement
  *   You have a University phone number (i.e., 621*/626*, NOT a cell phone) and do not need to keep it and/or do not need a headset.
  *   You do not have a University phone number but need a headset for internal Zoom use purposes (enter "None" for your phone number)

This includes you if you work remotely but your original phone or your departmental base is in Forbes, Marley, or Herring Hall.

This form is not for phones shared by a space (e.g., department or lab phone numbers) or call center numbers. (Managers for those areas should coordinate with Jennifer Garcia.)

Please make sure you fill out the form all the way to the final SUBMIT button.

UITS needs you to record:

  *   Your name and NetID
  *   Your University phone number (or "None")
  *   Whether you send or receive external calls for your work and will need a Zoom licensed phone. (If you use any of Zoom Phone's special services<https://it.arizona.edu/documentation/specialized-telephone-requirements>, you need to have a licensed phone.) If not migrated, it will be disconnected.
  *   Whether you need additional equipment (headset, desktop handset)
     *   See Zoom Phone Equipment<https://it.arizona.edu/documentation/zoom-phone-equipment> for information.

If you do not complete this form by September 6, UITS will not have your phone and equipment requirements and you will lose your phone number.

The transition of numbers from your current phone service to Zoom Phone is tentatively scheduled for October 19, 2022.

During the week prior to transition UITS will deliver all the requested equipment (headsets and desk handsets) to your building and building staff will arrange distribution.

On transition, there may be a short interruption in service during migration.

After transition, bring any old phone equipment you may have to the designated point in your building for collection.
