[Plantsci] University Compliance with Federal Vaccine Requirement

Hello All:

You all would have received the email<https://view.comms.arizona.edu/?qs=ffcc789da4c1eff2913707d1b8c89f778636bd336aae2ef4ffaaf801be2705e6c083c1ade6821c736e0293e4c0166711d1176c7aa90c0329a19855e6b2a55cfe776b750c760e7cc091d84552f2a55a70> from President Robbins last week concerning the requirement for ALL EMPLOYEES to be fully vaccinated by December 8th. All employees are required to upload their vaccine documentation<https://health.arizona.edu/covid-19-vaccine-document-upload> by the deadline showing they are fully vaccinated<https://www.cdc.gov/coronavirus/2019-ncov/vaccines/fully-vaccinated.html?s_cid=11350:cdc%20guidance%20fully%20vaccinated:sem.ga:p:RG:GM:gen:PTN:FY21> unless they have been granted a religious or disability accommodation. Few things:


  *   All Employees means all Faculty, Staff, Postdocs, Graduate Assistant/Associates and Student Employees. DCC's are not included at this time.
  *   Fully Vaccinated means:
     *   2 weeks after the second dose in a 2-dose series, such as the Pfizer or Moderna vaccines
        *   Last day for 1st dose- 10/27; Last day for 2nd dose- 11/24 to meet 12/8 deadline
     *   2 weeks after a single-dose vaccine, such as Johnson & Johnson's Janssen vaccine
        *   Last day for single dose- 11/24 to meet 12/8 deadline
  *   Employees please double check your document was uploaded by the deadline, especially if you have already done it. You have two options to verify:
     *   Use your NetID to log into Wildcat OneStop<https://onestop.wellcheck.arizona.edu/>. If you see a green "VX" below your picture, your upload was successful and you have fulfilled the vaccination requirement.
     *   You may also log into the vaccine upload system<https://vaccine.wellcheck.arizona.edu/> to verify your upload status.
  *   Accommodations:
     *   Request based on disability or medical condition would go through the DRC: https://drc.arizona.edu/covid/employees-vaccine-accommodation
     *   Request based on religious beliefs will go through UA-Human Resources: Complete this form<https://hr.arizona.edu/sites/default/files/COVID-19%20Religious%20Accommodation%20Form.pdf>, then email to HR-COVID19-Questions at email.arizona.edu<mailto:HR-COVID19-Questions at email.arizona.edu>.
  *   Supervisors: Please do not make any lists or record who has or who has not been fully vaccinated. The University will find the best way to track those who have not uploaded their vaccines while keeping HIPAA and FERPA compliant. This information will be shared when it comes out.
  *   All new hires will be required to upload proof of their vaccine from Dec. 8th and on.
  *   The University will address a failure to comply with the vaccine requirement consistent with the employee conduct and behavior policies in the applicable UHAP, Classified Staff and University Staff<https://policy.arizona.edu/human-resources> handbooks.
