[Plantsci] FW: UA BookStores- Change in Interdepartmental Billing to Service Billing

Hello All,

 

There is a new form to use to shop at the UA Bookstores for valid items for
your labs.  But, please not this form requires my signature and name as I am
the Fiscal Officer for the School of Plant Sciences.  Therefore, please
submit this form to the Admin Office for my signature.

The University of Arizona BookStores

 

RE:                       All Interdepartmental Billing Services

 

Due to the implementation of the new UAccess financial system on October
5th, Service Billing will be used to process interdepartmental billings for
department purchases in lieu of IDB.  

The Service Billing form can be obtained from the UA BookStore website at:
http://uabookstores.arizona.edu/forms/SB

Please complete and print the form, have it signed by the Department Fiscal
Officer, and bring it to the Customer Service Counter at UA BookStores,
SUMC, AHSC, McKale Sports Stop, Flandrau Science Center, UA South, or UA
Downtown for making purchases.  We will need the following information on
the Service Billing Agreement:

1.     Department account number

2.     Quantity and description of item(s) being purchased (Please specify
the allowable maximum limit on the purchase in case the specified amount may
be adjusted for sales taxes.)

3.     Department contact name and phone number 

4.     Fiscal Officer's signature

concerns regarding this information.
