[CED/CD/EAT] Stakeholder Paper & Pencil Surveys with Script

Hello CEDs,

I have attached the surveys that should be used for collecting 100 random
paper & pencil surveys in each county.  The script is also included with
the survey in both English and Spanish.

Please mail your completed surveys to me, at the address below.  You may
over collect, if you wish.  Please complete collection by mid-January.

Also, please ensure that you have sent out the electronic version of the
survey to "users" and "non-users" (expert representatives).  The return has
been VERY SLOW!  You might want to send out an email reminder.
