Hello UAAC -

I hope registration season is going well for you and your departments. There are two updates I would like to share with you from the Office of the Registrar.

Authorization for Release of Information Form

The Authorization for Release of Information form has been updated. There are two updates that are important to share with the campus community.

*         Long-term use is now set for 5 years so students do not have to renew their authorization annually.

*         We no longer require the purpose for the authorization on the form.

Administrative Change of Schedule Form

The Change of Schedule form moved online in spring 2020. The updated process is student initiated and cannot be used by departments. There is a new online form for Administrative Change of Schedule requests for departmental errors and exception processing. The form is located on the Change of Schedule Website<https://www.registrar.arizona.edu/cos%20> under Information for Academic Departments.

The Administrative Change of Schedule form can be used for the following actions:

*         Late add

*         Re-enrollment in a class

*         Drop without W grade

*         Swap without W grade

The Administrative Change of Schedule request can be used by any member of an academic department including academic advisors and departmental staff. The Office of the Registrar will review each request for accurate information and for an appropriate explanation before an exception is approved.
