Greetings Agvisors:

Happy Wednesday!  There is a new online process for General Petitions for Extenuating Circumstances for Medical Reasons for retroactive complete medical withdrawals. As of October 1, 2019 paper medical petitions will not be accepted.

All other University General Petitions, non-medical related, will continue to be processed through CAS advising office prior to sending to the university committee.

Students may be referred to speak to a CAS college advisor on either petition.  Let me know if you have any questions.  Thanks team!

Subject: Online Medical Petitions

Hello everyone,
If a student wishes to file a General Petition for Extenuating Circumstances for Medical Reasons the paper retroactive complete medical withdrawals are now available on UAccess Student Center. As of October 1st, we will no longer accept paper medical petitions.
Please advise your students to use the online process by:

1.    Logging into their UAccess Student Center

2.    Going to the "other academic" scroll tab,

3.    Selecting, "Retroactive Medical Withdrawal"

4.    Selecting the TERM

  1.  Write or upload a personal statement (500-word limit).

·         Your statement should describe the nature of your mental or physical illness, injury or disability, the duration of the condition, and the challenges that you faced in completing your coursework.

6.    Upload a letter from your licensed health care provider. Documentation must include a letter from your licensed health care provider on letterhead which includes:

     *   The approximate date of onset of the mental or physical illness or injury or disability giving rise to the circumstances surrounding the request for a complete retroactive withdrawal for extenuating circumstances, and the dates through which such condition(s) continued.
     *   The general nature of the mental or physical illness or injury or disability and how and why it warrants the action sought.
  1.  Submit the petition for processing.
  2.  Instructor Statements
     *   All instructors will be contacted by email for attendance and grading information after the student submits their petition. Physical instructor statements will not be required for retroactive medical withdrawals.
