Greetings Agvisors,

Updated FERPA form below. In case you may not be aware, when a student signs the form in your office you can scan it, upload it on Advisor Notes for your records and then forward original form via campus mail to the Administration Building room 210 if the student wants it to be officially flagged with a service indicator on UAccess. It’s good for up to one year.  This would prevent the student having to fill a form out each time it may be needed.

If the student chooses to just use the form that one time they are meeting you then the steps above (aside from uploading it on Advisor Notes) would not be necessary.

The Authorization For Release of Information form has been revised and is now available on the Registrar’s website under FERPA Forms at this link: https://www.registrar.arizona.edu/online-forms-available-from-office-of-the-registrar#266.

The form has been changed to request a student to select a security word or number for a second party or parties to identify themselves:

Many times students give access to agents (parents, individuals, sponsors, organizations, law offices) and university offices have not had a way of identifying the agent (over the phone for example), unless they are physically present. The security word or number will be  useful to identify callers or it’s something you can use in case you’re unsure of the person’s identity. The security word or number will be included in the Comments section of the Service Indicator page when the form is processed by the Registrar’s Office.

As a reminder, if your department accepts an authorization form directly from a student, you should send it to the Registrar’s Office for processing. We add the CIR service indicator to the record that permits any department within the university to view who has access and to what information. Parents, for example, will typically contact several departments for information to assist their student, so it’s important for the service indicator to be present on the record.
