[Agvisors] Electronic Change of Major and Student Exception Forms

Hello Agvisors,

Hope your semester is off to a great start!

Attached are the two electronic forms that were introduced at the Agvisors meeting last week.

The student exception form (green sheet) should be used for any changes to a student's advisement report.  Please fill out the form as you would the paper one.  If you are using a transfer course please indicate the name of the transfer course in addition to the model and group number.  When you save the form please do so in this format: Last Name_First Name_Date (example: Smith_John_8-26-13).  Remember that you must use dashes in the date instead of slashes.  Please email the exception form directly to me (kgeary at email.arizona.edu<mailto:kgeary at email.arizona.edu>) and be sure to copy the student on the email as well (this will act as their signature).

The change of major form should be used to add any CALS major, minor, or option to a student's plan of study.  Please fill out the form as you would the paper one.  When you save the form please do so in this format: Last Name_First Name (example: Smith_John).  Please email the change of major form directly to Maria Bueno (mbueno at email.arizona.edu<mailto:mbueno at email.arizona.edu>) and be sure to copy the student on the email as well (this will act as their signature).
