ASSOCIATE DIRECTOR-ANR SEMINAR FOLLOW UP AND EVALUATION FORM

Dear Colleagues,

 

As follow up to Ursula Schuch's seminar presentation today for the Associate
Director-ANR position, and in readiness for Paul Brown's presentation
tomorrow, if you or anyone in your unit accessed (or will access) the
seminars through the webcast, we have a Personal Interview Form (or
evaluation) that we would appreciate receiving from each participant.

 

The form can be accessed at:

downloaded and printed, and once completed, can be faxed (520-621-1314),
scanned and sent by email (to scrofts at cals.arizona.edu) or dropped off in
Forbes 301.  There is an option to sign the form but if you do send by email
and it is unsigned, it will be printed out, the email deleted, and your
input will remain anonymous.
