Hello All,

We need the attached release form signed by everyone in the School who wants
to have their picture posted on the new PLS  (SPLS) website.   This policy
and procedure went into effect in 2011.  That is why your photos are allowed
on the old website without this release but not on the new website.

Therefore the new website cannot go-live until we have a release form from
every employee on file.

Please turn in your signed form to me in Forbes 303.
