<p class="MsoNormal">Per our conversation on Tuesday, I’m including the link to submit your Phase Proposals below. It has some very basic fields which are consistent with what we’ve been discussing. The longer-text fields are for you to provide any relevant
data points, trends, or external factors for EAT to consider in your requests. You’ll find the format of the form very similar to the monthly report mechanism you’re used to.<o:p></o:p></p>

<p class="MsoNormal">There are also 3 File Attachment fields at the bottom of the form, should you need them. For the most part, many of you have been including any charts/diagrams into one document and submitting. Either method works. Upon submission, you
will receive a confirmation e-mail with a brief summary of your request/proposal. It will also include a link to access should you need to make any changes/edits to what you’ve submitted.<o:p></o:p></p>
