A revised Proposal Routing Sheet (PRS) form and instructions are now
available from the Sponsored Projects Services (SPS) web site:<br>

<b>Please note that, for all newly-routed proposals, SPS will begin
requiring the use of this revised PRS form on Tuesday, May
29<sup>th</sup>.</b> Proposals routed prior to that date, which
have not yet been signed by SPS, will <b><i>not</i></b> need to be
re-routed using the revised PRS form. <br>
<br>
Below is a list of revisions to the PRS form and instructions:<br>
· Data entered into the
PRS form will remain when you save a local copy to your computer (Adobe
Reader 8.0 or higher)<br>
· <b>FRS ACCOUNT</b>
replaced by <b>Account Number<br>
</b>· <b>Proposal
Type</b> revised to include the following, helpful descriptions for
<b>Renewal</b>, <b>Continuation</b>, and <b>Revision</b>:<br>
o Renewal (competing)<br>
o Continuation (non-competing, previously routed)<br>
o Revision (continuation/supplement, not yet routed)<br>
· <b>Foreign Nation</b>
question revised as follows:<br>
o <b>Collaboration w/Foreign Nation(s): </b>If any portion of
the project is to be completed with the assistance of<br>
collaborators in a foreign nation, check the ‘Yes’ box and enter the
foreign nation name(s) in the space<br>
provided. This data is collected for the Vice President for
Research.<br>
· <b>Export Control
Compliance</b> section added:<br>
o <b>Foreign national project personnel (including UA
Personnel)?</b>: Check the ‘Yes’ box if any<br>
foreign persons (includes a foreign person located outside the U.S. or UA
foreign national<br>
personnel) will work on the proposed project. Check the ‘TBD’ box if it
is not yet known if any<br>
foreign personnel will work on the proposed project. Note: A “green card”
holder (permanent<br>
resident) has the same status as a U.S. citizen.<br>
o <b>Travel to foreign nation(s)?: </b>Check the ‘Yes’
box if it is expected that foreign travel will be required to complete
the proposed project, and list the name(s) of the foreign nation(s) on
the line below. Note: Review the International Travel section of the
Export Control web site for travel restrictions to certain
embargoed/sanctioned countries<br>
o <b>Will any equipment (including laptops and/or
smartphones), technology, or software be taken<br>
outside the U.S.?</b>: Check the ‘Yes’ box if any equipment, technology,
or software will be taken<br>
outside the United States for the proposed project.<br>
o <b>Will you be working on a U.S. military base abroad?</b>:
Check the “Yes” box if you will be working<br>
on a US military base abroad. Note: This question is required per the
Defense Base Act (DBA);<br>

· <b>Subaccounts</b>
replaced by <b>Account Distribution </b>in F&A Allocation Box on page
two of PRS (<b><i>does not change how this distribution method
works</i></b>):<br>
o If you select account distribution, the F&A
distribution % will be “info only”. The quarterly F&A
Allocation will be based on actual account activity/distribution.<br>
· <b>EIN</b> replaced by
<b>UA NetID</b>:<br>
o Please include the NetID for all individuals listed in the
Principal Investigator Summary. If an<br>
individual has not yet been hired by the University, please complete
& submit the Person of Interest<br>
(POI) Designation Form so that a NetID/EmplID and other access services
can be established. The form<br>
can be found on the Human Resources website at
