This is your annual reminder to provide this document to any new members of external advisory groups associated with your unit/county/center/program and maintain copies of the signed document in your files.

  *   If your unit/county/center/program has an external advisory group not listed below, please let me know so I can add them to our central list. If you can provide me with the number of group members, their purpose and the current chair’s name/email, this would be very helpful in keeping our records up to date.
  *   If any of the groups listed below have disbanded, please let me know so I can update our central list.
