[CED/CD/EAT] Extension Agent Performance Reviews

Good afternoon,

As you're completing your faculty's performance reviews, here's a couple of quick reminders regarding this form:

  *   For the Overall Rating (bottom of page 3), please enter one of the options provided for the previous 8 sections:
     *   Truly Exceptional
     *   Meets/Exceeds Expectations
     *   Needs Improvement
     *   *Unsatisfactory - *NOTE: if you score your faculty as Unsatisfactory, the university's remediation plan is triggered.  You can find the requirements of those plans (Faculty Development Plan (FDP) or Performance Improvement Plan (PIP)), here UHAP 3.2.05<https://policy.arizona.edu/faculty-affairs-and-academics/annual-performance-reviews-faculty>.
  *   Completed forms can be sent to me (drodriguez1 at arizona.edu<mailto:drodriguez1 at arizona.edu>).
