Information regarding the paper & pencil surveys is being requested by our
analysis team.  They will use this information for writing up the report.
Please send me the following information:


   - How were the paper & pencil surveys collected?
   - Who collected the paper & pencil surveys?
   - Where were the paper & pencil surveys collected?
   - Were any incentives given to the persons collecting the surveys?
   - Were any incentives given to respondents?
