Students have a Dean of Student’s hold because they have not been reading their university email. It is the students responsibility to check their university email at least several times a week. The Dean of
Student’s Office is not the only department around campus that corresponds with a student using their university email. If students do not prefer to use their university email, the student needs to forward their university email to a preferred email they
do use.

Numerous emails have been sent to the student regarding the Title IX Training starting in January. Our last correspondence was a letter sent to them through their university email from
DOS-Codes@email.arizona.edu giving them again additional time to do the 10 minute Title IX Training.

Title IX holds have an IX in the description. Dean of Student holds should be directed to the Dean of Students Office at 520-621-7057. There are several reasons students would have hold from our office. When
they have a negative service indicator, the indicator will tell them who to contact to take care of the hold.

Title IX Training is a federally mandated program. Students that have been matriculated into the system will need to complete the training and there is a deadline. Each semester new students will have to do
this training. If the student does not complete by the deadline they have a hold put on their account until it is completed and they are charged a $50 monetary sanction since Failure to Comply is a code

When a student calls our office regarding the Title IX hold, we do advise them where to find the video and instructions for the certificate to show completion. Again, students would be aware of the training
and how to do the training if they read their university email.

Title IX Training is on the Student UAccess TO DO LIST. They click on the “Title IX Training” it opens another hyperlink. They click on this and it opens the 10 minute video. Students are to take a screen
shot of their certificate and send it to our office. The Training will drop off their To Do List the following business day but we do not know a student completes without correspondence from them. Students can bring in their laptop into the office and
we can physically show them the steps to complete the training.

The Title IX training is part of the new students orientation. I have not been informed on how advisors, faculty, departments or Residence Life were advised of this training. I’m sure with increased awareness

Subject: DOS service indicator description - Title IX

Dear Terri:

Good afternoon. I work in the College of Agriculture and Life Sciences general advising unit and we have been receiving inquiries from students on a DOS hold on their UAccess accounts. The negative service indicator prevents them from
enrolling in courses and only refers them to contact the DOS unit.

It seems the indicator has to do with Title IX, based on student feedback (after contacting their department advisor, then college advisor, then DOS). Would it be possible to reference Title IX within the service indicator description
or provide the instructions on how to proceed with the training completion? Students seem to be referred to multiple locations and/or individuals prior to finding out how to proceed. Advisors also do not know what the DOS hold is in reference to. One student
reported to have called the DOS number and was referred to the Title IX website to complete the training without further instructions.

It is just a suggestion to try and make the process clear for students and advisors (to refer appropriately). Thank you in advance for considering.

I am available to discuss this further over the phone at 621-3611. Initially I contacted the Title IX unit and was referred to you to assist with the systemic piece.
