I am attaching the new Change of Schedule (COS) form that is effective
immediately for Fall 2014 and reflects the new drop and withdrawal policies.
Please destroy all other copies/supplies of the form that exist throughout
your department and use only this version. The Registrar's website has been
updated with the new form:  http://registrar.arizona.edu/registrarforms.

 

The information below will help to clarify the changes and how the form is
to be used.

 

1)      Grade Column:  Undergraduate students will no longer receive E
grades. They will be able to drop courses through the 10th week of the term
without instructor or dean's permission and will automatically receive a W
grade after the 2nd week.  The form will still be used to grade Graduate and
Professional Students with an E or W grade. The grade column now indicates
that it is only for these groups of students. 

 

2)      Dean's offices and academic advisors are still expected to work with
their students to adjust their maximum unit load on UAccess Student on the
Term Activation page, Enrollment Limit tab as before. This is not
accommodated on the COS form.

 

3)      There is a new box on the form for Academic Department use. This box
will be used by departments to request that a student not be penalized with
a W grade in certain situations:

 

.         In the past the Registration and Transcripts Office has switched
students from one section of a course to a different section of the same
course without a W grade. We will continue this practice.

 

.         The department may now check a box and sign to indicate that a
student is not enrolled in the correct level of a course (i.e. math,
language, honors courses) and may ask for a switch to a more appropriate
level without a W grade resulting. The student will bring the COS form to
the Registration and Transcripts Office for processing.

 

.         The department may now check a box, sign and explain that it made
an error by enrolling the student in the wrong section of a course and ask
for a switch to the correct section without a W grade resulting. The student
will bring the COS form to the Registration and Transcripts Office for
processing. 

 

4)      For Late Registration:  The COS form will still be used for adds
throughout the semester--instructor signatures for adds will be required
after 2 weeks, and dean's signatures will be required after 8 weeks as
before. For now, this late registration box will also be used for dean's
offices to indicate approval for Late Change Petitions after the 10th week
of classes. It will not be necessary for dean's offices to provide the Late
Change Petition documents to the Registration and Transcripts Office.
