We are working on revising the change of major form.  I have attached the draft of the form we will be putting into use soon, but before it is distributed at next week's agvisors meeting your feedback would be greatly appreciated.  Please send any comments or suggestions to me before Monday afternoon so your feedback can be included in the final form.
